United Physical Therapy

We treat our patients gently, with respect, and provide the social and physical support they need to help them achieve their functional goals. We strive to empower patients to have an active role in their recovery, and are advocates of patient education. 

We provide a well trained staff with each therapist working one-to-one with each patient. 

 

 

Our therapists continue to gain professional skills and are selected with care focusing on their high skills and commitment to continued professional growth.

Our focus at United Physical Therapy is to work with our patients as active participants-united together with each therapist- improving our patient's ability to self manage their health problems.

We provide timely and informative feedback to referral sources and believe communication between referral source, clinician, and patient is crucial to successful rehabilitation and appropriate case management. 

Referral sources receive:

• Quick and convenient scheduling

• Initial evaluation and plan of care report

• Progress notes every 30 days: including progression towards functional and objective goals

A copy of the patient's home exercise program or gym exercises (upon request).
